Application
- Online Application Form. All applicants, internal or transfer, can use this form to apply for the Biology major. Visit our Admissions page for more information.
Registration and sign up
- BBIO 498/499 Registration Request: Once you have discussed your research or independent study plans with your instructor, fill out this form to request registration for the credit. Please allow up to two weeks for processing.
- Symposium Presentation Sign up: If you want to use something other than BBIO 495 (Investigative Biology) for the Investigative Biology requirement, you must present your research! Use this form to sign up for a research symposium event (always on Friday of finals week each quarter).
- Course Repeat Request: If you have taken a course twice or more, you must complete this form to request registration for that course.

Can I submit a petition after the deadline to be reviewed for the current quarter?
We will only allow late submissions for emergencies and extraordinary circumstances such as a death in the family, a serious illness (cold and flu excluded), or injury with proof provided and the petition review must be determined as necessary for graduation or the upcoming registration cycle. If you believe this applies to you, please reach out to the biology advisor. All applications regardless of extenuating circumstances must be approved by the biology advisor by Wednesday of week 3 of the quarter for our advisor and faculty to have enough time to review your petition prior to the decision deadline.
Investigative Biology Petition
Investigative Biology Petition: This form is for petitioning alternative research experiences to count towards the investigative biology requirement. ?This form is NOT used for BBIO499 registration requests, you can find the BBIO 498/499 Registration Request form above under registration and sign up. Visit the Investigative Biology page for more information.

- B BIO 495 (Investigative Biology). This is the default option. You do not need to petition for this class.
- B BIO 499 (5 credits Undergraduate Research) or other Biology-related research at UW Bothell.*
- 5 credits of Undergraduate Research at another institution, such as UW Seattle.*
- At least 150 hours of a research internship, job, or similar experience.*
*Using any experience other than completing B BIO 495 will require additional steps. Please thoroughly read and complete all required steps found on the Investigative Biology page.

Biology Major & Minor & Neuroscience Minor Petitions
Please fill out a separate form for each requirement you are petitioning:
- Biology Major & Minor Petition: Use this form if you would like to petition any core or elective course for your Biology Major/Minor. Do not use this form for petitioning; Introductory Biology, Chemistry, Physics, or General Education courses, instead, send the biology advisor a copy of the syllabus directly.
- Neuroscience Minor Petition: Use this form if you would like to petition the core or elective requirements for the Neuroscience Minor. Do not use this form for petitioning Introductory Biology, send the biology advisor a copy of the syllabus directly.
Documentation: Petitions require supporting documentation. You may need to include additional documentation if your course syllabus does not include the information below. Petitions with incomplete documentation will be declined during the initial review period.
Checklist for Petition Syllabi- If your syllabus does not include this information please submit additional documentation with any missing information:
- Course instructor name (s) and contact information
- Dates and times of course instruction (Example Spring 2016, T &Th 11:15‐1:15)
- Number of days/hours per week the class met
- Number of course credits (ex: 1 credit)
- Course format (i.e in-person, hybrid, online)
- Prerequisites for the course
- Texts and readings
- Schedule of topics covered in the class
- How was your learning assessed?
- Was there a lab associated with this course? If so, then the lab schedule and topics should be included.
Other information:
- Petition Decisions are based on course content and equivalency; “graduating on time” is not considered when making petition decisions.
